Electronic Child Registration Form

Please register on the Child Pilot app prior to dropping off your child for their first visit. Once you create an account, you will receive an invitation to fill out your family profile on Child Pilot. Once this has been completed, you can make reservations/appointments directly through the Child Pilot app.
